HDC offers over 28 programs focusing on mental health and recovery services.
Local counties contract with HDC to provide case management services to adults and children with serious mental health issues. Case managers work closely with individuals and families to help them access needed supports and services to maximize independence, mental health, and community integration.
Youth Targeted Case Management is ongoing monitoring and care coordination for individuals under 18 years old in need of assistance. Case Management connects families to community resources to help meet their basic needs, provides advocacy and support to those with barriers to develop a personalized plan for success. With a focus on the individual’s educational, behavioral, medical, mental health needs and social skill building. This service is beneficial for individuals who have difficulty accessing or sustaining services independently.

CSP outreach services are provided to adults with the primary goal of assisting consumers in recovery from mental illness.
Interventions include supportive therapy, medication management, education regarding mental illness, assistance with learning needed skills, finding and maintaining affordable housing, independent living skills, benefits assistance, vocational support, symptom monitoring, outreach, crisis assistance, psycho-social rehabilitation, and general case management. Program services are primarily provided on an outreach basis with services typically occurring in the home or community.
We believe homelessness is not a choice and, given the opportunity for safe, affordable housing, everyone prefers a permanent place to live. HDC’s Homeless Program serves individuals in Duluth who are homeless, have a mental illness, and may have health problems and substance abuse issues. We provide community outreach to locate people experiencing homelessness and assist them in obtaining and keeping permanent housing. We also provide case management, employment assistance, transportation, benefits assistance, and crisis intervention.
