Children’s Case Management
Youth Targeted Case Management is ongoing monitoring and care coordination for individuals under 18 years old in need of assistance. Case Management connects families to community resources to help meet their basic needs, provides advocacy and support to those with barriers to develop a personalized plan for success. With a focus on the individual’s educational, behavioral, medical, mental health needs and social skill building. This service is beneficial for individuals who have difficulty accessing or sustaining services independently.
**Must have a form of Wisconsin MA to qualify for services.
Community Support Program (CSP)
CSP outreach services are provided to adults with the primary goal of assisting consumers in recovery from mental illness.
Interventions include supportive therapy, medication management, education regarding mental illness, assistance with learning needed skills, finding and maintaining affordable housing, independent living skills, benefits assistance, vocational support, symptom monitoring, outreach, crisis assistance, psycho-social rehabilitation, and general case management. Program services are primarily provided on an outreach basis with services typically occurring in the home or community.
Crisis Services (non-mobile)
We offer in-person, walk-in crisis support Monday through Thursday from 8:00 a.m. to 4:30 p.m., and Fridays from 8:00 a.m. to 3:00 p.m. During these hours, you can also reach us by phone for immediate crisis assistance.
After hours, phone support is available for urgent crisis situations only. Please note: this line is not a therapy service or a warm line for general support.
While some crisis services in Minnesota offer mobile outreach, our program does not provide in-person mobile crisis response.
Project Reach Out
Project Reach Out works to improve the network of support to youth and their families through confidential and voluntary crisis intervention referral services. Each year, teens run away for a variety of reasons that may include unresolved conflict, lack of communication, and differences over roles, rules, and expectations.
Project Reach Out can help during these difficult times by ensuring access to 24-hour crisis intervention services, reuniting youth with their families when appropriate, encouraging the resolution of inter-family problems through mediation and other services, strengthening family relationships, encouraging stable living conditions for youth, and helping youth decide upon a constructive course of action and decision making in a non-judgmental atmosphere.
