Your initial appointment with HDC can take around two hours.
The first part of your appointment will be with a care coordinator for initial assessments. Then, you will spend about an hour and a half with a therapist to do the initial evaluation for services. This is for all HDC clients regardless of what program you will be receiving services in.
To make this process as smooth as possible, we ask that you fill out the following forms ahead of time and bring them to your appointment. You can print these off or stop by one of our offices for a paper copy. If you are unable to complete these forms before your appointment, they will be provided to you at the time of your scheduled appointment.
